Health Waiver?

According to our records you have not waived the Student Health Insurance Plan
charge on your University bill. The deadline to submit a waiver is October 20,
2006. You may waive participation in the plan if you have comparable insurance
coverage. All waivers must be submitted on line, to access the waiver function
go to SPIRE and click the 'Health Insurance Waiver' or go to
[url]www.chickering.com[/url], click on 'Student Connection', then 'Find Your School'.
You will be prompted for your policy number - enter 10016. If you wish to
participate in the plan, you do not need to respond to this letter, you will
automatically be enrolled as long as you are taking 5 or more credits. If you
have recently completed an on line waiver and have a confirmation number,
please disregard this letter.

It is very important that you waive participation in the plan by the deadline
if you do not want the coverage. You will NOT be able to waive the spring 2007
plan coverage if you are enrolled for fall 2006.

For further information on the Student Health Insurance Plan and Waiver,
Contact University Health Services Enrollment at 413-577-5192.

  IMPORTANT INFORMATION FOR GRAD GEO MEMBERS

GEO eligible Graduate students are entitled to a 95% reduction of their
student health insurance costs for a single plan. To exercise this option you
can call the Bursar's office at 413-545-2368 or visit the Bursar's office 215
or 225 Whitmore Building Monday through Friday between the hours of 9 AM and 5
PM. GEO insurance waivers are not subject to the October 22 deadline. If you
are a GEO eligible Grad and want to use your outside coverage please follow
the instructions outlined above by October 22.

i also recieved this email it seems that doesn't matter with us. We are GEO member and we have 95% deduction. The insurance waiver means you don't want insurance from school and instead you have your own insurance.

I chose Program Fee this semester (with full RA), is there anything special for me to do for the health insurance? I haven't got any bills related with health insurance  on my Bursar's account.

Yes, you need to go to UHS asap to enroll your health insurance.  Students taking less than 5 credits are not automatically enrolled into the health insurance.  The deadline should be the last day of add/drop, but it should be ok.  Just go asap.  You need to pay $63.50 for 1 semester coverage, which is 5% of total cost.  You got 95% waived.

I already get enrolled this Aug. because I select Family plan starting from Aug. 1st. At that time, I chose "I'll take >5 credits this semester". The problem is now I am paying program fee without credits. Should I do something for that change?

If you have already paid $63.5 for your individual plan in Aug, you should be all set.  The safest way for you would be calling UHS and make sure they have your name in their system.
